ABSTRACT

As a new technology with unique drug delivery advantages, nanoemulsion has been widely used in the field of traditional Chinese medicine (TCM) preparations. By searching, classifying and sorting out the literature reports at home and abroad in recent years, this paper systematically expounded the application advantages and production mechanism of nanoemulsion in delivering effective components of TCM from three aspects of improving oral bioavailability, enhancing targeting effect and delaying drug release. The current formulation optimization strategies, preparation processes and quality evaluation indicators commonly used in TCM nanoemulsion were summarized. Based on the research status of TCM nanoemulsion with different active components, the common problems and possible solutions in the development of TCM nanoemulsion were discussed, and the future research hotspots and directions of TCM nanoemulsion were prospected. This article clarifies the feasibility of nanoemulsion for enriching the selection of TCM dosage forms, which can provide reference for the subsequent rational design and improvement of TCM preparations. At the same time, it is revealed that the research focus of TCM nanoemulsion in the future lies in the integrated research of TCM compounds, and shows a trend of multi-disciplinary joint and targeted research.

ABSTRACT

Objective To investigate the effect of the correction of the position error of the abdominal tumor by cone beam scanning. Methods Six patients with abdominal tumor were selected with double-blind method, and electronical portal imaing device was used to collect the information of the patient dosimetry during radiotherapy.EDOSE tool was applied to 3D dose reconstruction so as to execute real-time 3D dose verification of the treatment plan. Each patient had 5 sets of data acquired when image guidance existed and another 5 sets when image guidance was absent. The treatment effects with or without image guidance were compared.Results The overall γ passing rate with cone beam CT image guidance gained no advantages over that without image guidance, though the setup error was corrected to some extent. Image guidance contributed nothing to increasing target area coverage while decreasing exposure dose to normal tissue. Conclusion Setup error correction by the image guidance based on bone marker has no significant effect on improving 3D dose verification and dose accuracy during the radiotherapy of the abdominal tumor patient.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the operative management and surgical techniques for lumber disc herniation (LDH) in aged patients (>or=65 years).</p><p><b>METHODS</b>From 2005 to 2010,the data of 43 patients with LDH underwent surgical treatment were retrospectively analyzed. There were 25 males and 18 females,aged from 65 to 70 years old with an average of 67.6 years. The course of disease was from 6 weeks to 7 years with an average of 10.2 years. Fenestratiodn discectomy or extended fenestration discectomy and unilateral or bilateral fenestration were used according to the conditions of location,type of herniated macleos polposus and nerve root compression. Among the patients,the nerve root canal was enlarged,hyperplastic osteophyma and soft tissue were removed, bilateral articular process was reserved in order to maintain the stability of the lumbar segment.</p><p><b>RESULTS</b>All the patients were followed up from 1.2 to 5.2 years. According to the criteria of HU you-gu, 25 cases got excellent results, 15 good, 2 fair and 1 poor. No infection or nerve injury was found. Nerve function of patients had a normal or near normal recovery.</p><p><b>CONCLUSION</b>For the treatment of LDH in aged patents,it is key that reasonably choose the operative method, completely remove the hyperplasy of diseased region and enlarge the nerve root canal, thoroughly loose the nerve root.</p>

ABSTRACT

<p><b>BACKGROUND</b>The diagnosis of Pneumocystis pneumonia (PCP) in immunocompromised patients is still challenging today due to the absence of an in vitro culture system and the low diagnostic accuracy of microscopic examinations. Herein, we performed a meta-analysis to evaluate the accuracy of real-time polymerase chain reaction (PCR) in the diagnosis of PCP.</p><p><b>METHODS</b>We searched Web of Knowledge and Medline from 1990 to May 2010 for studies reporting diagnostic accuracy data regarding the use of real-time PCR in the diagnosis of PCP in immunocompromised patients.</p><p><b>RESULTS</b>Ten individual studies were included. Overall, the sensitivity of real-time PCR was 97% (95%CI: 93% - 99%); the specificity was 94% (95%CI: 90% - 96%). The area under the HSROC curve (95%CI) for real-time PCR was 0.99 (0.97 - 0.99). In a subgroup analysis regarding studies involving HIV patients among the study population, the sensitivity and specificity were 97% (95%CI: 93% - 99%) and 93% (95%CI: 89% - 96%), respectively. Regarding studies using Bronchoalveolar lavage (BAL) samples only: sensitivity = 98% (95%CI: 94% - 99%); specificity = 93% (95%CI: 89% - 96%), respectively. Regarding studies using microscopy as a reference standard: sensitivity = 97% (95%CI: 92% - 99%); specificity = 93% (95%CI: 88% - 96%). However, high between-study statistical heterogeneity was observed in all analyses.</p><p><b>CONCLUSIONS</b>Real-time PCR has a good diagnostic accuracy and may provide a useful adjunctive tool for the diagnosis of PCP in immunocompromised patients. Further studies are needed in order to identify any differences in the diagnostic performance of real-time PCR in HIV and non-HIV immunocompromised patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate effect of the treatments and prognostic factors of patients with pulmonary metastasis from colorectal cancer.</p><p><b>METHODS</b>Clinical data of 79 patients who suffered from lung metastatic diseases from colorectal cancer in 1990 - 2010 were retrospectively analyzed. The number of patients who had received lung operation was 22, and non-operated group contained 57 patients. Compared the prognosis of operated group and non-operated group and analyzed the prognostic factors.</p><p><b>RESULTS</b>The median survival time after the pulmonary resections was 34.5 months; the overall survival of 1-, 3- and 5-year survival rates were 90.9%, 45.4% and 4.5%, and the overall of 1-, 3-, and 5-year survival rate in non-operated group were 59.6%, 14.0% and 0. The surgery (RR = 4.805, 95% CI: 1.864 - 12.384, P = 0.001) and the number of metastasis (RR = 2.177, 95% CI: 1.431 - 3.314, P = 0.010) were the factors that could influence the patients prognosis.</p><p><b>CONCLUSION</b>The surgery for pulmonary metastases from colorectal cancer is effective.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the differences in oncologic outcomes between inflammatory adhesion and malignant adhesion in patients with stage IIC colorectal cancer after multivisceral resection(MVR).</p><p><b>METHODS</b>A retrospective review was undertaken of 287 patients who underwent MVR for stage IIC CRC, 120 patients for stage IIB, and 140 patients for IIIA. Patients were divided into two groups: inflammatory adhesion(IA) and malignant invasion(MI).</p><p><b>RESULTS</b>There were 153 patients with colon cancer and 135 patients with rectal cancer in the stage IIC group. The overall survival was significantly lower in the MI group at 5 years(38.5% vs. 59.4%, P<0.05). Stage IIC patients with IA had similar survival rate to the patients with stage IIB CRC. Compared to the MA group, patients with stage IIIA CRC showed significant differences in 5 years overall survival rate. Univariate analysis showed that differentiation, adhesion pattern, and complication were significant prognostic factors for patients with colon cancer, while pathological characteristics, adhesion pattern, and differentiation were significant for rectal cancer.</p><p><b>CONCLUSIONS</b>MI is an adverse prognostic factor for patients with stage IIC CRC. T4 should be further classified according to the adhesion pattern.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical characteristics and treatment of Fournier's gangrene.</p><p><b>METHODS</b>We retrospectively analyzed the clinical data of 23 cases of Fournier's gangrene, all with flare and pains in the scrotum and penis and different degrees of involvement of the scrotum, penis, perianal area, perineum, inguinal and lower extremities. The patients were treated by early debridement, incision-drainage, anti-infection and hyperbaric oxygen therapy, respectively. Scrotoplasty was performed for 11 of the cases, penile and scrotal dermatoplasty for 7, and penile amputation and urethral fistulation for 2 with penile necrosis. One of the cases underwent suprapubic cystostomy, and another 1 received colostomy.</p><p><b>RESULTS</b>Twenty of the patients were recovered and 3 (1 with diabetes and 1 with AIDS) died after surgery.</p><p><b>CONCLUSION</b>Fournier's gangrene is a fatal disease. Early diagnosis and timely surgical intervention are essential for the management of the disease.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the presentation of a neutralization epitope-containing peptide antigen of hepatitis E virus (HEV) on chimeric virus-like particles (VLPs) of hepatitis B surface antigen (HBsAg).</p><p><b>METHODS</b>The gene fragment corresponding to amino acids (aa) 551-607 (HEnAg) of HEV capsid protein, which contains the only neutralization epitope identified to date, was fused via a synthetic glycine linker in frame with the gene of HBsAg. The resulted fusion gene was then integrated through transformation into the genome of Pichia pastoris under the control of a methanol-induced alcohol oxidase 1 (AOX1) promoter and expressed intracellularly. The expression products in the soluble cell extracts were characterized by Western blot, ELISA, CsCl density gradient analysis, and electron microscopic visualization.</p><p><b>RESULTS</b>The novel fusion protein incorporating HBsAg and the neutralization epitope-containing HEnAg was expressed successfully in Pichia pastoris with an expected molecular weight of approximately 32 kD. It was found to possess the ability to assemble into chimeric HBV/HEV VLPs with immunological physical and morphological characteristics akin to HBsAg particles. Not only did the chimeric VLPs show high activity levels in a HBsAg particle-specific ELISA but they were also strongly immunoreactive with hepatitis E (HE) positive human serum in a HEV specific ELISA, indicating that HEnAg peptide fragments were exposed on VLP surfaces and would be expected to be readily accessible by cells and molecules of the immune system. Similarity between chimeric VLPs to highly immunogenic HBsAg particles may confer good immunogenicity on surface-displayed HEnAg.</p><p><b>CONCLUSION</b>The chimeric HBV/HEV VLPs produced in this study may have potential to be a recombinant HBV/HEV bivalent vaccine candidate.</p>
